All of the services we offer work comprehensively and holistically, addressing a multitude of issues depending on the needs of the affected individual and their family. All services are free of charge, regardless of income or insurance status.

Care Coordination

One of our Care Coordinators will help you connect with community resources  for which you are eligible and entitled based on your circumstances. Additionally, your Care Coordinator will act as an advocate on your behalf and provide guidance and other resources as needed.

Peer Mentoring

A person who has sustained a life altering injury similar to yours will assist with guidance, support and education on your journey ahead.

Caregiver Support

CONNECT + ABILITY at Warm Springs staff will assist a family member or caregiver with workshops and other resources to ensure the Caregiver remains healthy, both emotionally and physically, to assist their loved one on their journey with a disability.

Professional Development

We offer quarterly workshops and symposiums for those professionals working with people with life altering injuries and their families to grow their skills and continue providing excellent care.


Who are our services for?

We work in Bexar County and surrounding areas, any income or insurance status is eligible. We are currently providing services for people with:

  • Traumatic Brain Injury
  • Spinal Cord Injury
  • Amputation
  • Stroke

How does it work?

Our experienced Care Coordinators act as your advocate, and guide you to the support and services that you need. We are currently accepting referrals for any eligible individual. To make a referral, please call us at (210) 819-4325.
